    This PDF prints out with some sort of strange pattern at the the top instead of the arrows that appear on the screen.
    It is not my printer or computer as it did it at work as well.
    Anybody else mention this?

    If you’re talking about the arrows before and after the “Vehicle Permit No.” and “Do not forward this form” lines I just downloaded and printed and it worked fine.
